OFCCP Disability Self-Identification Form Approved

The Office of Management and Budget (OMB) has approved the continued use of the Office of Federal Contract Compliance Program (OFCCP) disability self-identification form. This form, labelled CC-305, has been approved for use through July of 2029. Approval for use of this form expired in April of 2026, but OMB extended the use of the form each month and has now approved the use of the disability self-identification form by OFCCP through 2029.
Use of CC-305 is required under OFCCP's regulations associated with Section 503 of the Rehabilitation Act of 1973. Federal contractors and subcontractors are required to use CC-305 any time information on disability status is requested from applicants or employees. The form must be offered to:
Applicants during the pre-offer stage of the employment process
Applicants during the post-offer stage of the employment process
Employees at least once every five years
Routine information collections by federal agencies must have an OMB number and an expiration date. The version of CC-305 available from the OMB website does not have the July 2029 expiration date. As of the original publication date of this blog, the version of CC-305 available from the OFCCP website showed a June 30, 2026 expiration date. However, OFCCP has now has the version of CC-305 with the July 2029 expiration date available.
In July of 2025, OFCCP had proposed major changes to its Section 503 regulations that would have eliminated the use of form CC-305 and ended data collection and reporting requirements regarding individuals with disabilities. The Department of Labor's regulatory agenda states that these efforts are still moving forward. However, the approval of form CC-305 suggests that OFCCP may no longer be seeking to make major changes to its Section 503 regulations.
